Research framing: NAD+ turnover releases nicotinamide (NAM), which is cleared largely by nicotinamide N-methyltransferase (NNMT) attaching a methyl group to produce methylnicotinamide for excretion. That methyl group comes from S-adenosylmethionine, so sustained NAD+ or precursor loading increases demand on the methionine cycle. Vitamin B12, alongside folate, is central to regenerating SAM through that cycle, which is the stated rationale for co-administering it. This is a mechanistic argument rather than a demonstrated clinical outcome: there is no controlled trial showing that adding B12 to an NAD+ protocol improves results.
