Murepavadin

Also known as: POL7080, RG7929, Inhaled murepavadin

Investigational macrocyclic peptide antibiotic for Pseudomonas aeruginosa. IV Phase 3 trials were halted in 2019 over kidney injury. Not approved.

Mechanism of Action

Murepavadin is a synthetic beta-hairpin peptidomimetic modeled on the host-defense peptide protegrin I. It binds LptD, an outer-membrane protein that transports lipopolysaccharide to the cell surface in Pseudomonas aeruginosa, blocking outer-membrane assembly. This target is specific to Pseudomonas, so it has a very narrow spectrum and little activity against other bacteria.

Half-life
Not well established in patients; a few hours IV in healthy volunteers
Administration Routes
Intravenous, Inhaled
Research Status
Clinical

Reported Benefits

  • Studied for hospital-acquired and ventilator-associated pneumonia caused by Pseudomonas aeruginosa (IV, halted)
  • Inhaled formulation studied for chronic Pseudomonas lung infection in cystic fibrosis and bronchiectasis
  • Potent in-vitro activity against multidrug-resistant Pseudomonas isolates

Potential Side Effects

  • Acute kidney injury (rates higher than expected in IV Phase 3 trials)
  • Infusion-related symptoms in early trials
  • Long-term safety not established

Common Dosing

No approved dose. Phase 1 studies tested single and multiple IV ascending doses and, later, inhaled doses in healthy volunteers; the IV Phase 3 regimens were stopped. Use outside a clinical trial is not appropriate.

Contraindications

  • Investigational: not established
  • Kidney impairment was the key safety concern in IV trials

No. Polyphor paused its IV Phase 3 trials in 2019 after higher-than-expected rates of acute kidney injury. Spexis reported a completed Phase 1 study of inhaled murepavadin in early 2023, but as of 2026 we could not confirm any active later-stage trial, and the CF Foundation lists the program as discontinued.

Its target, the LptD outer-membrane transporter, has a structure in Pseudomonas aeruginosa that murepavadin binds selectively. This narrow spectrum could spare normal flora but means it would not treat mixed infections on its own.
