Ghrelin is a 28-amino-acid peptide that, in its acylated form, binds the growth hormone secretagogue receptor (GHS-R1a) in the hypothalamus and pituitary. Receptor activation stimulates phospholipase C signaling, raising intracellular calcium and driving robust pulsatile growth hormone secretion. Centrally, ghrelin activates NPY/AgRP neurons in the arcuate nucleus to increase appetite and promote energy storage. It also influences gastric motility and reward pathways, making it a central regulator of energy homeostasis.
