Clomiphene blocks estrogen receptors at the hypothalamus and pituitary, removing negative feedback so the body secretes more GnRH, LH, and FSH — which raises endogenous testosterone and supports spermatogenesis. It is a mixture of two isomers: enclomiphene (trans, the anti-estrogenic driver) and zuclomiphene (cis, longer-acting and more estrogenic), which is why side-effect profiles differ from pure enclomiphene.
